SOGD clarification of dungeon building

I'm not clear on how it works. Is there a run through for dungeon building in raid and standard scenarios?

Are tiles placed as models move into them? What's the mechanic for this?

How do raids work..... it states that defenders can place rooms anywhere on the board. How do you ensure links? I'm just a bit in the dark over the whole thing

As the models move through an exit or other connection, the player places down a tile as he likes.
